The best single-axis rating module
This is a great module for single-axis rating.
Despite the name, it is possible to change the rating scale to something other than 5. And if you don't want stars, there are a number of other symbols to choose from (you can even create your own set).
The module also allows you to assign labels or meanings to each star level. This helps users pick a suitable rating, and improves rating consistency.
The scope of this module is somewhat limited. There is no official support for multi-axis rating, and it doesn't allow for text reviews.

Fivestar is a nice little module
I used this on my d5 site vegan columbus and it's been very useful. I like the multiple star image options and it's great that I can use it with views to make a "top rated" list.

Virtually perfect
Jeff Eaton did a spectacular job on this one. It's very customizable and relatively easy to use.

One of the most useful, well thought out, and nicely designed modules
Fivestar really is the ratings module for Drupal. Now that you can make a rating required for comments, it has become the "simple review module" that people have been looking for as well. Lots of configurable options. I've never found myself saying "well gee, if I could only tweak it like XYZ"... it always seems to do what I want. It's very well maintained and will only keep getting better as more stars are contributed.

Absolutely great voting module
Very flexible, neat & easily configurable. Has a highly responsive interface (Ajax based). Very well maintained (new features like multi-axis voting nearly finished). If you need a voting widget, search no more - Fivestar is the way to go.
I didn't rate it as 5/5 in terms of features since currently there is no simple up/down voting. With this additional feature I would call it an ultimate Drupal voting solution.
